MSP BillBudBilling cardioversion under sedation: cardioversion code or resus time?
The scenario
68F in rapid AFib with chest pain. You cardiovert under procedural sedation (8 min sedation). Evening shift.
How to bill it
Two options (Level 2 shown). (A)
Y33025 cardioversion $105.70 at 100% + Level 2 evening (01822) at 50% = $51.44, total $157.14. Sedation codes can't be billed alongside cardioversion. (B) Level 2 evening $102.88 + 01870 resus (20 min = 4 units × $31.56 = $126.24) = $229.12; resus is added in full, not halved. With a Level 2 or 3 code, resus wins once you are at the bedside more than 5 minutes (2 units: $102.88 + $63.12 = $166.00). If documentation supports Level 3 (01823 $128.14), the totals are $180.99 and $254.38.What to watch for
Cardioversion has a special rule about sedation codes. Consider the alternative.
